This form is intended to communicate to the Care Team the needs of people who call Mountain View Church home and are experiencing a challenging season. Mountain View Church offers a wide range of resources to help guide individuals through difficult circumstances and we are here to help.

A member of the Care Team will reach out to the individual in need of care & support to assess their needs and determine how we may best provide assistance. 

All information submitted through this form will be treated with the utmost confidentiality and will be shared only with the Care Team and pastoral staff.
